Transition Point

A significant milestone on a student’s educational journey. These are critical markers that not only indicate student success at a point in time, but predict future success at the next transition point. The Transition Point indicators below are marked with the Cradle to Career Arrow icon. 

Systems Indicator

A Systems Indicator is a qualitative or quantitative measurement that shows how inequities in outcomes, resources, decision-making power, and opportunities are distributed within groups and/or geographic areas. Their purpose is to inform policies and practices within systems, institutions, organizations, and programs. Systems Indicators can be used in combination with individual-level indicators to hold systems, organizations, and institutions accountable to create the conditions that are necessary for all children, youth, and their families to thrive, no matter their race, place, education, or income level. Systems Indicators below are marked with the compass icon. 

Social Determinant of Health

Per Healthy People 2030*, Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. SDOH indicators are marked below with the SDOH wheel icon.

Summit Education Initiative (SEI) annually reports Summit County educational attainment along with the critical transition points on the Cradle to Career Continuum. The PDF is the Summit County Educational Attainment Scorecard. The Scorecard data are from Summit County schools.
